Petaluma Health Center Data Breach Investigation

WHAT HAPPENED?

Recently, Petaluma Health Center reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on December 15, 2025, Petaluma Health Center’s electronic medical record system OCHIN notified Petaluma Health Center that an unauthorized individual gained access to one of the systems belonging to TriZetto, a third-party company that works with OCHIN.1 As a result, TriZetto launched an investigation to determine the nature of the incident.

Through its investigation, TriZetto confirmed to Petaluma Health Center that sensitive personal information in TriZetto systems related to Petaluma Health Center patients may have been accessed by an unauthorized third party during the breach. As a result, Petaluma Health Center began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Contact information
  • Health-related or insurance information

 

As a result of the breach, Petaluma Health Center began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, Petaluma Health Center and TriZetto are providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
