CommuniCare+OLE Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating OLE Health, which does business as CommuniCare+OLE (“CommuniCare+OLE”), regarding its recent data breach. The CommuniCare+OLE data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

ABOUT OLE HEALTH D/B/A COMMUNICARE+OLE:

CommuniCare+OLE is a healthcare organization based in California. Founded in 1972, CommuniCare+OLE is the result of merger in 2023 between OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County.2,3 Through this merger, CommuniCare+OLE offers a range of healthcare services to patients, including pediatrics, reproductive health, family medicine, dental care, behavioral health, mobile medicine services, and more.2 Headquartered in Napa, California, CommuniCare+OLE employs over 500 individuals and offers services across over 10 locations in California.3,4

WHAT HAPPENED?

Recently, CommuniCare+OLE reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on December 15, 2025, OCHIN, CommuniCare+OLE’s electronic medical record system provider which works with a third-party company named TriZetto, notified CommuniCare+OLE that an unauthorized individual gained access to one of TriZetto’s systems.1 As a result, CommuniCare+OLE launched an investigation to determine the nature of the incident.

In the sample breach notice provided to the Attorney General of the California, CommuniCare+OLE does not elaborate further on the nature of the security incident that impacted TriZetto systems. On behalf of CommuniCare+OLE, OCHIN and TriZetto began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Contact information
  • Health-related or insurance information

 

As a result of the breach, CommuniCare+OLE began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, CommuniCare+OLE is providing affected individuals with [a list of the specific types of sensitive information impacted.
