VillageCareMAX Data Breach Investigation

ABOUT VILLAGECAREMAX:

VillageCareMAX is a healthcare organization based in New York. Founded in 1977, VillageCareMAX specializes in providing a range of healthcare and long-term care services to older adults and those with other chronic diseases and conditions, including healthcare plans, behavioral health, dental care, social services, in-home care, transportation services, and more.2,3,4 Headquartered in New York, New York, VillageCareMAX employs over 1,000 individuals and provides care and services for more than 35,000 individuals annually through its health plan and community programs.3

WHAT HAPPENED?

Recently, VillageCareMAX reported to the Attorney General of the Commonwealth of Massachusetts that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, TMG Health, Inc. (“TMG”), a third-party administrator for healthcare providers which VillageCareMAX engaged to provide call center services, discovered that an unauthorized individual had access to VillageCareMAX information stored within TMG’s information system.1,5 As a result, TMG launched an investigation to determine the nature of the incident.

Through its investigation, TMG confirmed to VillageCareMAX that sensitive personal information in TMG systems related to VillageCareMAX patients may have been accessed by an unauthorized third party between November 20, 2024 and September 19, 2025. As a result, VillageCareMAX began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Member number
  • Health information

 

On January 13, 2026, VillageCareMAX began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Massachusetts residents, VillageCareMAX is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
