Wyandot Center Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Wyandot Behavioral Health Network, which does business as Wyandot Center (“Wyandot”), regarding its recent data breach. The Wyandot data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

Wyandot is a community mental health center based in Kansas. Today, Wyandot provides a range of mental health and recovery services to adults, addressing needs related anxiety, trauma, substance abuse, life transitions, and more.2 As a part of the Wyandot Behavioral Health Network, Wyandot is a branch of a unified system of care made up of 4 interconnected organizations addressing mental health and social needs in Kansas City, alongside PACES, RSI, and Kim Wilson Housing.2,3 Headquartered in Kansas City, Kansas, Wyandot Behavioral Health Center employs over 200 individuals.4

WHAT HAPPENED?

Recently, Wyandot announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, Wyandot discovered unusual activity on its systems.1 As a result, Wyandot launched an investigation to determine the nature of the incident.

Through its investigation, Wyandot confirmed that sensitive personal information and protected health information in its systems may have been accessed or acquired by an unauthorized third party between September 21 and September 22, 2025. As a result, Wyandot began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On November 5, 2025, Wyandot completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Patient ID
  • Medical record number
  • Health insurance information
  • Service date
  • Diagnosis/condition information
  • Provider name
  • Prescription information
  • Medical history information

 

On November 19, 2025, Wyandot posted notice of the breach on its website. Based on the website breach notice, Wyandot is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
