Personic Health Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Personic Management Company LLC, which does business as Personic Health (“Personic”), regarding its recent data breach. The Personic data breach involved sensitive personal information and protected health information belonging to over 10,000 individuals.

Personic is a physician-owned healthcare service provider based in Virginia. Founded in 2020, Personic specializes in advanced wound care and debridement, offering mobile in-home care and assistance.3,4 Headquartered in Vienna, Virginia, Personic serves patients in twelve states and employs over 200 individuals.4,5

WHAT HAPPENED?

Recently, Personic reported to the Attorney General of New Hampshire that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on September 1, 2025, Personic was alerted to unauthorized activity involving a third-party software platform it uses to process patient information.1 As a result, Personic launched an investigation to determine the nature of the incident.

Through its investigation, Personic confirmed that sensitive personal information and protected health information on the third-party platform was accessed and acquired by an unauthorized actor on August 29, 2025. As a result, Personic began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Medical information (e.g., date of service, diagnostic treatment information, medical record number, medical history, medications, patient number, provider name, surgical information, and treatment location)
  • Insurance information

 

On November 18, 2025, Personic began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to New Hampshire residents, Personic is providing affected individuals with a list of the specific types of sensitive information impacted and 24 months of complimentary credit monitoring services.
