Strauss Borrelli PLLC, a leading data breach law firm, is investigating Sapphire Community Health, Inc. (“SCH”) regarding its recent data breach. The SCH data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.
ABOUT SAPPHIRE COMMUNITY HEALTH INC.:
SCH is a primary care medical services provider based in Montana. Today, SCH offers various services such as comprehensive primary care, acute care for minor injuries and illnesses, sports physicals, and substance use disorder counseling.2 Headquartered in Hamilton, Montana, SCH employs over 25 individuals and has an additional location in Hamilton, Montana.
WHAT HAPPENED?
Recently, SCH announced that it’s electronic health record vendor, Cerner Corporation (“Cerner”) had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, in late February Cerner became aware of a security event in which an unauthorized third party gained access to protected health information on legacy Cerner systems.1 As a result, SCH launched an investigation to determine the nature of the incident.
Through its investigation, SCH confirmed that sensitive personal information in its systems may have been accessed by an unauthorized third party as early as January 22, 2025. As a result, SCH began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
 - Social Security number
 - Date of birth
 - Medical record numbers
 - Other information used in providing care
 
On October 22, 2025, SCH posted notice of the breach on its website. Based on the breach notice, SCH is providing affected individuals with a list of the specific types of sensitive information impacted and 24 months of complimentary credit monitoring services.
