WHAT HAPPENED?

Recently, Cookeville Regional discovered that it had experienced a data breach in which the sensitive protected health information in its systems may have been accessed. On September 12, 2025, Cookeville Regional filed official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights.1 Though much information is still not known about the Cookeville Regional breach, the U.S. Department of Health and Human Services’ reporting guidelines require entities to report data breaches when they involve protected health information. Therefore, it is likely that the Cookeville Regional breach included the protected health information belonging to an undetermined number of individuals. As a result of the data breach, Cookeville Regional may soon begin notifying individuals whose information may have been impacted.
