CardioVascular Health Clinic Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating CardioVascular Health Clinic (“CHC”) regarding its recent data breach. The CHC data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

ABOUT CARDIOVASCULAR HEALTH CLINIC:

CHC is a healthcare facility based in Oklahoma. Founded in 2015, CHC specializes in the early prevention, diagnosis and treatment of heart and vascular conditions.2 Today, the services provided by CHC include heart care, peripheral artery disease treatment, prostate artery embolization treatment, diagnostic testing and treatment, fibroid treatments, neuropathy treatments, peripheral venous treatments, knee replacement alternatives, vein care, and more.3  Headquartered in Oklahoma City, Oklahoma, CHC has over 10 locations across Oklahoma, and employs over 50 individuals.

WHAT HAPPENED?

Recently, CHC announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on March 4, 2025, CHC experienced a network disruption.1 As a result, CHC launched an investigation to determine the nature of the incident.

Through its investigation, CHC confirmed that sensitive personal information in its systems may have been accessed by an unauthorized third party between February 18, 2025, and March 4, 2025.1 As a result, CHC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Phone number
  • Email address
  • Address
  • Date of birth
  • Driver’s license/state ID number
  • Financial account information
  • Medical information (e.g., treatment/diagnosis information, prescription information, provider name, medical record/case number)
  • Medicare/Medicaid ID number
  • Health insurance information
  • Treatment cost

 

As a result of the data breach, CHC posted a notice of the incident on its website. Additionally, CHC plans to send breach notification letters to impacted individuals. Based on the website breach notice, CHC is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
