Brainard Surgery Center Data Breach Investigation

Recently, Brainard announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on February 23, 2025, Brainard became aware of suspicious activity in its computer network.¹ As a result, Brainard launched an investigation to determine the nature of the incident.

Through its investigation, on January 30, 2025, Brainard confirmed that sensitive personal information in its systems may have been accessed and acquired by an unauthorized third party during the breach. As a result, Brainard began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

• Name
• Social Security number
• Address
• Date of birth
• Driver’s license number or other state identification number
• Medical claims information
• Clinical information (including diagnosis/conditions, medications, and additional health information)
• Health insurance information

As a result of the breach, Brainard posted a notice of the incident on its website. Additionally, Brainard plans to send breach notification letters to impacted individuals.
