Alternate Solutions Health Network Data Breach Investigation

Recently, ASHN announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on or around May 30, 2024, an unauthorized actor obtained access to an ASHN email account.¹ As a result, ASHN launched an investigation to determine the nature of the incident.

Through its investigation, ASHN confirmed that sensitive personal information in the email may have been accessed and acquired by an unauthorized third party during the breach. As a result, ASHN began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

• Name
• Social Security number
• Date of birth
• Address
• Driver’s license number
• Physician and clinical names
• Clinical information
• Diagnostic information and treatment information

As a result of the breach, ASHN posted a notice of the incident on its website. Additionally, on April 14, 2025, ASHN began sending breach notification letters to impacted individuals.
