EncompassCare is a therapy and rehabilitation organization based in Ohio.2 Today, EmcompassCare provides therapy services to patients in skilled nursing centers, assisted living centers, as well as in a patients home in conjunction with home health services.2 Additionally, EncompassCare is part of a larger system of senior care communities managed by HCF Management, Inc., which offers care communities in Ohio and Pennsylvania and assisted living residences in Ohio.3 Headquartered in Lima, Ohio, EncompassCare employs over 1,000 individuals.
WHAT HAPPENED?
Recently, EncompassCare reported to the Attorney General of the Commonwealth of Massachusetts that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on October 3, 2024, EncompassCare learned that a third party had gained access to certain of its management company’s computer systems.1 As a result, EncompassCare’s management company launched an investigation to determine the nature of the incident.
Through its investigation, EncompassCare confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party on September 17, 2024. As a result, EncompassCare began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social security number
- Date of birth
- Financial account number
- Medical information
- Health insurance information
On January 9, 2025, EncompassCare began mailing data breach notification letters to impacted individuals. Based on the breach notices provided to the Massachusetts residents, EncompassCare is providing affected individuals with a list of the information impacted and 24 months of complimentary identity monitoring services.
