BayMark is a healthcare and substance abuse treatment provider based in Texas. Today, BayMark is one of North America’s largest providers of medication-assisted treatment (MAT) for substance use disorders.3 Accordingly, BayMark’s service offerings include opioid treatment programs (OTP), office-based opioid treatment (OBOT) practices, outpatient and inpatient detox services, residential treatment centers, and behavioral health services.3 Headquartered in Lewisville, Texas, BayMark has over 200 locations across the United States and employs over 1,000 individuals.
WHAT HAPPENED?
Recently, BayMark announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. According to the breach notice, on October 11, 2024, BayMark learned of an incident that disrupted the operations of some of its IT systems.1 As a result, BayMark launched an investigation to determine the nature of the incident.
Through its investigation, BayMark confirmed that sensitive personal information in its systems may have been compromised by an unauthorized third party between September 24, 2024, and October 14, 2024. As a result, BayMark began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Medical/clinical information (e.g., services received, date of service, treating provider, treatment and/or diagnostic information)
- Insurance information
As a result of the data breach, BayMark posted a notice of the incident on its website. Additionally, on January 8, 2025, BayMark began mailing breach notification letters to affected individuals. Based on the website breach notice, BayMark is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
