Today, Daleville Dental offers a comprehensive list of general, restorative, and cosmetic dental services to individuals and families.3 These services include cleanings and exams, dental implant restoration, porcelain veneers, root canal treatment, dentures and partials, teeth whitening, tooth-colored fillings, porcelain crowns, and more.3 Daleville Dental is headquartered in Daleville, Alabama.

WHAT HAPPENED?

Recently, Daleville Dental announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on November 7, 2023, Daleville Dental became aware of unusual activity within its computer network. As a result, Daleville Dental launched an investigation to determine the nature of the incident.

Through its investigation, on November 10, 2023, Daleville Dental discovered that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party on or around November 7, 2023. As a result, Daleville Dental began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On October 28, 2024, Daleville Dental completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license or state identification number
  • Military identification number
  • Passport number
  • Financial account information
  • Payment card information
  • Medical information
  • Health insurance information

 

As a result of the data breach, Daleville Dental posted a notice of the incident on its website. Additionally, on November 21, 2024, Daleville Dental mailed notices to affected individuals. Based on the website breach notice, Daleville Dental is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
