Axis Health System Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Southwest Colorado Mental Health Center, Inc., which does business as Axis Health System (“Axis Health”), regarding its recent data breach. The Axis Health System data breach may have involved sensitive personal identifiable information belonging to an undetermined number of individuals.

Axis Health is a private, nonprofit healthcare organization based in Colorado. Founded in 1960, Axis Health provides a variety of services to individuals, including primary care, care coordination, community services, crisis care, dental care, and pharmacy services.2 Additionally, Axis Health offers a multitude of mental and behavioral health services such as individual and group therapy, psychiatric assessment and medication management, drug and alcohol monitoring, and substance use treatment.2 Headquartered in Durango, Colorado, Axis Health has ten additional locations across Colorado and employs over 200 individuals.

WHAT HAPPENED?

Recently, Axis Health reported to the Attorney General of the Commonwealth of Massachusetts that it had experienced a data breach in which the sensitive personal identifiable information in its systems may have been accessed and acquired. According to the breach notice, on August 26, 2024, Axis Health became aware of irregular activity within its computer network. As a result, Axis Health launched an investigation to determine the nature of the incident.

Through its investigation, Axis Health learned that sensitive personal information in its systems may have been viewed and obtained by an unauthorized individual between July 9, 2024, and September 4, 2024. As a result, Axis Health began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number

 

On September 23, 2024, Axis Health began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Massachusetts residents, Axis Health is providing affected individuals with a list of the specific type of sensitive information impacted and 24 months of complimentary credit monitoring services.
