WHAT HAPPENED?
On or around August 9, 2023, Fellowship Village discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Fellowship Village determined that an unauthorized third party accessed this sensitive information between July 27, 2023, and August 9, 2023. On July 1, 2024, Fellowship Village began notifying individuals whose information may have been impacted. The type of personal information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Address
- Driver’s license or state ID number
- Financial account information
- Medical information (e.g., patient ID number, medical record number, diagnosis and treatment information)
- Health insurance information
