WHAT HAPPENED?
On June 11, 2023, Coastal Orthopedics discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Coastal Orthopedics determined that an unauthorized third party accessed this sensitive information between June 6 and June 11, 2023. As a result, Coastal Orthopedics began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license number
- Date of birth
- Address
- Financial information (e.g., account number, credit or debit card number)
- Medical information (e.g., patient identification number, medical record number, diagnosis information)
- Health insurance information
