WHAT HAPPENED?
Recently, PVE discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Through its investigation, PVE determined that an unauthorized third party accessed this sensitive information between February 19, 2024, and March 4, 2024. On June 7, 2024, PVE began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license or state-issued ID number
- Passport number
- Green card
- Medical information
