WHAT HAPPENED?

On December 4, 2021, one of Texas Tech University Health Sciences Center’s vendor partners, Eye Care Leaders, Inc., experienced a data breach in which the sensitive personal identifiable information and protected health information of Texas Tech University Health Sciences Center patients may have been accessed. On June 7, 2022, Texas Tech University Health Sciences Center began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Telephone number
  • Date of birth
  • Driver’s license number
  • Gender
  • Medical record number
  • Health insurance information
  • Medial information related to ophthalmology services
