WHAT HAPPENED?
On or around October 31, 2021, a vendor of Affiliated Foot & Ankle Center, Virtual Private Network Solutions, LLC, discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information belonging to Affiliated Foot & Ankle Center patients may have been accessed. Virtual Private Network Solutions, which does business as VPN Solutions, manages electronic patient records for Affiliated Foot & Ankle Center. On July 1, 2022, Dr. Scott H. Andrew d/b/a Affiliated Foot & Ankle Center began notifying patients whose information was stored on the Virtual Private Solutions network during that time. The type of information exposed includes:

- Social Security number
- Address
- Date of birth
- Diagnostic information
- Health insurance information
