WHAT HAPPENED?

On June 9, 2022, Atlantic Dialysis Management Services discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Atlantic Dialysis Management Services determined that an unauthorized actor may have gained access to this sensitive information. On August 5, 2022, Atlantic Dialysis Management Services began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Health insurance information
  • Medical information (e.g., diagnosis, prescription, and treatment information)
