Dr. Giovannone conducts medical rounds on ortho-rehab patients at its facility.3 For patients requiring additional medical care, such as oxygen, wound care, and IV management, Lynwood Manor offers a full range of skilled nursing services in a caring and supportive environment.3 The services and therapies provided by Lynwood Manor include physical, occupational, and speech therapies, short term rehabilitation with therapy services 7 days per week, 24/7 nursing and personal assistant care, IV therapy and feeding tube, electric stimulation, balance training, cognitive retraining, self-feeding training, respite care, and veteran services. Additionally, Lynwood Manor provides quality long-term care services for families who require the security of 24/7 supervision, with daily activities from Bingo to religious services stimulate the residents’ need for social interaction, and mental and physical stimulation.4 Located in Adrian, Michigan, Lynwood Manor has one location.
WHAT HAPPENED?
In September 2022, Lynwood Manor discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Through its investigation, Lynwood Manor determined that an unauthorized actor gained access to this sensitive information on July 12, 2021. In November 2022, Lynwood Manor posted a HIPPA substitute notice of the incident on its website and on June 7, 2024, Lynwood Manor began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

- Social Security number
- Medical record number
- Medicare number
- Clinical information
