WHAT HAPPENED?
On July 17, 2022, Neurology Center of Nevada discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. During its investigation, Neurology Center of Nevada determined that an unauthorized actor accessed this sensitive information between June 12 and July 17, 2022. On September 15, 2022, Neurology Center of Nevada began notifying individuals whose information may have been impacted. The type of information exposed includes:

- Social Security number
- Driver’s license number
- Date of birth
- Address
- Health insurance information
- Medical information (e.g., diagnosis and treatment information, lab results, and medication information)
- Gender
