WHAT HAPPENED?

Recently, Phoenix House Florida experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Phoenix House Florida determined that an unauthorized actor may have accessed this sensitive information at various times between July 13 and November 1, 2021. On October 19, 2022, Phoenix House Florida began notifying individuals whose information may have been impacted. The type of information impacted includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Date of birth
  • Health insurance information
  • Financial information (e.g., credit or debit card number)
  • Electronic signature
  • Medical information (e.g., diagnosis or treatment information, history)
