Strauss Borrelli PLLC, a leading data breach law firm, is investigating U.S. Vision, Inc. and its subsidiary, USV Optical, Inc., regarding its recent data breach. The U.S. Vision data breach involved sensitive personal identifiable information and protected health information belonging to an undetermined number of individuals.

U.S. Vision is an optometry and eye-care company that specializes in prescribing eyeglasses and contacts in addition to caring for general eye health. Founded in 1885, U.S. Vision began with a single eyeglass store and manufacturing facility dedicated to providing stylish and high-quality eyewear. In the 1970s, U.S. Vision rebranded to its current name and began a period of rapid expansion with increasing demand for its products. Today, U.S. Vision is known for its major contracts with large department stores around the United States. Headquartered in Blackwood, New Jersey, U.S. Vision services more than 360 locations and employs over 1,000 individuals.
WHAT HAPPENED?
On May 12, 2021, U.S. Vision’s subsidiary discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, U.S. Vision determined that an unauthorized actor may have accessed this sensitive information between April 20 and May 17, 2021. Furthermore, U.S. Vision concluded that the sensitive information of its affiliates, Nationwide Vision Center, Nationwide Optometry, and SightCare, may have also been exposed. On October 28, 2022, U.S. Vision began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Address
- Health insurance information (e.g., billing and claims information)
- Medical information (e.g., medical record number, dates of service, diagnosis or treatment information, prescription information, provider name)
- Medicare or Medicaid information
