On June 3, 2022, Three Rivers Provider Network discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems was accessed and acquired. Through its investigation, Three Rivers Provider Network determined that an unauthorized actor accessed this sensitive information through an employee email account. Furthermore, on October 15, 2022, Three Rivers Provider Network began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Address
  • Passport number
  • Health information
