WHAT HAPPENED?
In May 2023, Superior discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Superior determined that an unauthorized actor may have accessed this sensitive information between May 15 and May 23, 2023. As a result, Superior began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Address
- Driver’s license or state ID number
- Financial account information
- Payment card information
- Medical information (e.g., patient record information, medical diagnosis or condition information, medical treatment information)
- Health insurance information
