WHAT HAPPENED?

On May 22, 2023, Family Vision of Anderson discovered that it experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Family Vision of Anderson determined that an unauthorized actor may have accessed this sensitive information by launching a ransomware attack on May 21, 2023. On July 26, 2023, Family Vision of Anderson began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Social Security number
  • Driver’s license information
  • Address
  • Date of birth
  • Demographic data (e.g., gender)
  • Contact information (e.g., phone number, email address)
  • Health insurance information (e.g., health insurance company, health insurance number)
  • Medical information (e.g., medical record number, medical history, appointment date(s), name of provider, prescription information, eye scan information)
