Mosaic Mental Health Data Breach Investigation

Headquartered in Bronx, New York, Mosaic Mental Health serves over 2,000 patients annually and employs over 50 individuals.

WHAT HAPPENED?

On July 27, 2023, Mosaic Mental Health discovered that it had experienced a data breach in which the sensitive personal identifiable information in its systems may have been accessed. Through its investigation, Mosaic Mental Health determined that an unauthorized actor may have accessed this sensitive information on July 27, 2023. Recently, Mosaic Mental Health began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Health insurance information
  • Medical information (e.g., diagnosis codes, clinical data, progress notes and evaluations, and procedure codes)
