WHAT HAPPENED?

Recently, Precisely discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Precisely determined that an unauthorized actor may have accessed this sensitive information between August 13 and August 16, 2023. On November 20, 2023, Precisely began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Passport number
  • Medical information
  • Health insurance information
