The Meadowlark data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

Meadowlark is a not-for-profit senior living and wellness organization based in Kansas. Founded in 1980, Meadowlark provides a range of senior living and wellness services to residents, including independent and assisted living, memory and Parkinson’s programs, home health care, physician services, rehabilitation care, cancer care, and more.2,3 Headquartered in Manhattan, Kansas, Meadowlark employs over 500 individuals.4
WHAT HAPPENED?
Recently, Meadowlark announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on his website, Meadowlark discovered unauthorized access to our network occurred.1 As a result, Meadowlark launched an investigation to determine the nature of the incident.
Through its investigation, Meadowlark confirmed that sensitive personal information in its network may have been accessed and acquired by an unauthorized third party between about July 12 and July 21, 2025. As a result, Meadowlark began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On January 28, 2026, Meadowlark completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number or state identification number
- Other government identification number
- Financial account information
- Credit or debit card information
- Medical information
- Health insurance policy information
As a result of the breach, Meadowlark posted notice of the breach on its website. Based on the website breach notice, Meadowlark is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
