360 Dental Data Breach Investigation

Recently, 360 Dental announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on November 16, 2025, 360 Dental identified a computer security incident involving its internal server.¹ As a result, 360 Dental launched an investigation to determine the nature of the incident.

Through its investigation, 360 Dental confirmed that sensitive personal information in its systems may have been accessed by an unauthorized third party during the breach. As a result, 360 Dental began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

• Name
• Social Security number
• Address
• Date of birth
• Contact information (e.g., telephone number, email)
• Patient account or chart number
• Dental and clinical records (e.g., treatment history, clinical notes, x-rays, and diagnostic information)
• Insurance provider and member ID
• Appointment information
• Emergency contact information
