Harmony Health Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Harmony Health Medical Clinic and Family Resource Center (“Harmony Health”) regarding its recent data breach. The Harmony Health data breach involved sensitive personal information belonging to an undetermined number of individuals.

ABOUT HARMONY HEALTH MEDICAL CLINIC AND FAMILY RESOURCE CENTER:

Harmony Health is a health center based in California. Today, Harmony Health provides a range of health services to patients, including medical clinic services, mobile clinics, perinatal services, behavioral health treatment, optometry services, medical home care, and more.2 Headquartered in Marysville, California, Harmony Health employs over 10 individuals and operates 4 locations in California.3,4

WHAT HAPPENED?

Recently, Harmony Health reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information in its care may have been compromised. According to the breach notice, on December 12, 2025, Harmony Health was notified of a data security incident involving TriZetto Provider Solutions (“TPS”), a third-party insurance clearinghouse used by healthcare providers to process insurance eligibility verification transactions.1 TPS reported that on October 2, 2025, it became aware of suspicious activity involving a web portal use by some of its healthcare provider customers. As a result, TPS launched an investigation to determine the nature of the incident.

Through its investigation, TPS confirmed to Harmony Health that sensitive personal information in TPS systems related to Harmony Health may have been accessed by an unauthorized third party between November 2024 and October 2, 2025. As a result, on behalf of Harmony Health, TPS began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Contact information (address, telephone number, email address)

 

As a result of the breach, Harmony Health began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, Harmony Health and TPS are providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
