WHAT HAPPENED?
Recently, Galine Frye reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, Galine Frye discovered unauthorized access to its network occurred on or around October 17, 2025.1 As a result, Galine Frye launched an investigation to determine the nature of the incident.
Through its investigation, Galine Frye confirmed that sensitive personal information in its systems may have been accessed and acquired by an unauthorized third party during the breach. As a result, Galine Frye began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On December 10, 2025, Galine Frye completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license or other government identification number
- Bank account information
- Credit or debit card information
- Medical information
- Health insurance policy information
On December 15, 2025, Galine Frye began mailing data breach notification letters to impacted individuals and posted notice of the breach on its website. Based on the breach notice sent to California residents, Galine Frye is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
