Adaptive is an in-home health services company based in Texas. Today, Adaptive provides a range of in-home health services to patients, including physical therapy services, orthopedic rehab, specialty wound and infusion care, general nursing services, and more.2 Additionally, Adaptive serves approximately over 2,000 patients weekly and is used by a range of hospitals and health centers for in-home services, including UT Southwestern Medical Center, Houston Methodist Leading Medicine, Ascension Health, The University of Texas MD Anderson Cancer Center, St. David’s Health Care, etc.2 Headquartered in Richardson, Texas, Adaptive employs over 300 individuals.2
WHAT HAPPENED?
On January 5, 2026, Adaptive reported to the Attorney General of Texas that it had experienced a data breach in which the sensitive personal identifiable information in its systems may have been accessed by an unauthorized third party. As of January 6, 2026, details of the breach have not been made publicly available by Adaptive. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Driver’s license number
- Government-issued ID number (passport, state ID card)
- Financial information (account number, credit or debit card number)
- Medical information
- Health insurance information
- Date of birth
