MNHC is a health center based in California. Founded in 1967, MNHC provides a range of health services to patients, including teen clinic services, pediatrics, family dental services, optometry, adult and senior medicine, pharmacy services, behavioral health care services, and more.2,3 Additionally, MNHC offers a variety of community programs, including wellness groups, a resource center, youth programs, counseling and outreach programs, etc.2 Headquartered in San Francisco, California, MNHC employs over 50 individuals and offers services across 6 locations.3,4
WHAT HAPPENED?
Recently, MNHC discovered that it had experienced a data breach in which the sensitive protected health information in its systems may have been accessed. On December 19, 2025, MNHC filed official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights.1 Though much information is still not known about the MNHC breach, the U.S. Department of Health and Human Services’ reporting guidelines require entities to report data breaches when they involve protected health information. Therefore, it is likely that the MNHC breach included the protected health information belonging to over 3,700 individuals. As a result of the data breach, MNHC may soon begin notifying individuals whose information may have been impacted.
