Founded in 2012, Fyzical provides physical therapy treatment and programs to patients for a variety of conditions, including dizziness and vestibular disorders, balance and walking problems, orthopedic complications, neurological disorders, pelvic floor disorders, and more.2,4 Headquartered in Sarasota, Florida, Fyzical employs over 1,000 individuals and operates dozens of locations across the United States.2,3,5
WHAT HAPPENED?
Recently, Fyzical announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on December 9, 2024, Fyzical became aware of unusual activity in its email environment.1 As a result, Fyzical launched an investigation to determine the nature of the incident.
Through its investigation, Fyzical confirmed that sensitive personal information in the emails may have been accessed and acquired by an unauthorized third party during the breach. As a result, Fyzical began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On November 25, 2025, Fyzical completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license or state identification number
- Financial account information
- Credit card information
- Health insurance information
- Medical health information
On December 19, 2025, Fyzical posted notice of the breach on its website. Based on the website breach notice, Fyzical is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
