Today, Synergy offers a variety of healthcare and healthcare related services to individuals, including primary care, physical exams, treatment of both acute and chronic and illnesses, medication management, medication assisted treatment, medical marijuana certifications, and telemedicine services.2 Headquartered in Torrington, Connecticut, Synergy operates out of one location.
WHAT HAPPENED?
Recently, Synergy discovered that it had experienced a data breach in which the sensitive protected health information in its systems may have been accessed. On November 4, 2025, Synergy filed official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights.1 Though much information is still not known about the Synergy breach, the U.S. Department of Health and Human Services’ reporting guidelines require entities to report data breaches when they involve protected health information. Therefore, it is likely that the Synergy breach included the protected health information belonging to over 1,200 individuals. As a result of the data breach, Synergy may soon begin notifying individuals whose information may have been impacted.
