Sun Valley Surgery Center Data Breach Investigation

Recently, Sun Valley announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on September 3, 2025, Sun Valley became aware of a potential issue involving its information systems.¹ As a result, Sun Valley launched an investigation to determine the nature of the incident.

Through its investigation, Sun Valley confirmed that sensitive personal information and protected health information in its systems may have been accessed by an unauthorized third party during the breach. As a result, Sun Valley began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

• Name
• Social Security number
• Contact information
• Date of birth
• Driver’s license or state-issued ID information
• Passport or other governmental ID information
• Health information (health history, treatment and diagnosis information, explanation of benefits, health insurance information, medical record number or patient identification number)

As a result of the breach, Sun Valley posted notice of the data breach on its website. Based on the website breach notice, Sun Valley is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
