WHAT HAPPENED?
Recently, Mountain West announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on or around March 17, 2025, Mountain West experienced unauthorized access to several of its email accounts.1 As a result, Mountain West launched an investigation to determine the nature of the incident.
Through its investigation, Mountain West confirmed that sensitive personal information and protected health information in the emails may have been accessed and/or acquired by an unauthorized third party during the breach. As a result, Mountain West began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Financial account number
- Financial account access information
- Payment card number
- Payment card access information
- Passport number
- Electronic signature
- Medical information
- Health insurance information
As a result of the breach, Mountain West posted notice of the breach on its website. Additionally, on September 22, 2025, Mountain West began mailing data breach notification letters to impacted individuals. Based on the website breach notice, Mountain West is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
