Strauss Borrelli PLLC, a leading data breach law firm, is investigating CareTracker, Inc., which does business as Amazing Charts (“Amazing Charts”), regarding its recent data breach. The Amazing Charts data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

Amazing Charts is a medical practice administrative software company based in Rhode Island. Founded in 2001 and acquired by Harris Healthcare in 2017, Amazing Charts provides electronic health records software to independent medical practices, in addition to offering systems for managing medical practice operations, billing, population heath, patient engagement, telemedicine, and more.2,3 Medical areas that Amazing Charts specializes in assisting include urgent care, family medicine, pediatrics, primary care, etc.2 Headquartered in West Warwick, Rhode Island, Amazing Charts employs over 50 individuals.3
WHAT HAPPENED?
Recently, Amazing Charts reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on or about June 19, 2025, Amazing Charts identified unusual activity on certain systems within a network managed by one of its third-party service providers.1 As a result, Amazing Charts launched an investigation to determine the nature of the incident.
Through its investigation, Amazing Charts confirmed that sensitive personal information and protected health information in the network of the third-party service provider may have been accessed by an unauthorized third party between June 15 and June 19, 2025. As a result, Amazing Charts began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On October 29, 2025, Amazing Charts completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Medical treatment or diagnosis information
- Physician name
- Medical record number
- Health insurance information
On November 12, 2025, Amazing Charts began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, Amazing Charts is providing affected individuals with a list of the specific types of sensitive information impacted and 12 months of complimentary credit monitoring services.
