WHAT HAPPENED?
Recently, FWMEP announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on December 17, 2024, FWMEP discovered suspicious activity in its network.1 As a result, FWMEP launched an investigation to determine the nature of the incident.
Through its investigation, FWMEP confirmed that the sensitive personal information and protected health information in its network may have been accessed or acquired by an unauthorized third party between December 12 and December 17, 2024.1 As a result, FWMEP began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license or state ID number
- Passport number
- Date of birth
- Medical information
- Health insurance information
- Bank account number
- Payment or credit card number
On October 3, 2025, FWMEP posted notice of the data breach to its website. Additionally, on October 2, 2025, FWMEP began mailing data breach notification letters to impacted individuals. Based on the website breach notice, FWMEP is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.1 A link to the website breach notice is below.
