Reimagine Network Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Reimagine Network, formerly known as Rehabilitation Institute of Southern California (“Reimagine”) regarding its recent data breach. The Reimagine data breach involved sensitive personal information and protected health information belonging to over 4,700 individuals.

ABOUT REIMAGINE NETWORK:

Reimagine is a nonprofit organization based in California. Founded in 1950, Reimagine provides programs and services that support individuals with disabilities through learning, workforce development, health and wellness, and child development .1,2 Headquartered in Santa Ana, California, Reimagine employs over 200 individuals and operates services across multiple campus locations in Orange County.1,2

WHAT HAPPENED?

Recently, Reimagine posted notice to its website that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, Reimagine  experienced a network disruption on June 23, 2025.1 As a result, Reimagine launched an investigation to determine the nature of the incident.

Through its investigation, Reimagine has confirmed that sensitive personal information and protected health information may have been acquired without authorization. As a result, Reimagine began a review of the data to determine what information had been impacted as well as identify the specific individuals affected, which concluded on August 6, 2025. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Address
  • Phone number
  • Date of birth
  • Social security number
  • Diagnosis/conditions
  • Medications
  • Health insurance information

On August 28, 2025, Reimagine posted notice of the breach on its website. Furthermore, Reimagine filed an official notice of the data with the U.S. Department of Health and Human Services’ Office for Civil Rights on August 29, 2025. 4 Based on the breach notice shared on its website, Reimagine is providing affected individuals with a list of the specific types of sensitive information impacted.
