WHAT HAPPENED?
Recently, Covenant Physician Partners discovered that it had experienced a data breach in which the sensitive protected health information in its systems may have been accessed. On May 28, 2025, Covenant Physician Partners filed official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights.1 Though much information is still not known about Covenant Physician Partners breach, the U.S. Department of Health and Human Services’ reporting guidelines require entities to report data breaches when they involve protected health information. Therefore, it is likely that Covenant Physician Partners breach included the protected health information belonging to over 88,000 individuals. As a result of the data breach, Covenant Physician Partners may soon begin notifying individuals whose information may have been impacted.
