Millennium is a home healthcare provider based in Oklahoma. Today, Millennium provides in-home healthcare to independent seniors, recovering patients, individuals with complex treatment plans, and those with specific needs such as a disability or chronic disease.2 Headquartered in Tulsa, Oklahoma, Millennium has seven branch locations in Oklahoma.
WHAT HAPPENED?
Recently, Millennium announced that the sensitive personal identifiable information in its care may have been accessed and acquired. According to the breach notice, on January 20, 2025, Millennium became aware of a security incident whereby an unauthorized third party gained access to two of its computer servers.1 As a result, Millennium launched an investigation to determine the nature of the incident.
Through its investigation, Millennium confirmed that sensitive personal information in its systems belonging to patients may have been viewed and obtained by an unauthorized third party on January 16, 2025. As a result, Millennium began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Phone number
- Medical conditions and care received
On March 19, 2025, Millennium posted a notice of the incident on its website. Additionally, Millennium plans to send breach notification letters to impacted individuals. Based on the website breach notice, Millennium is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
