Chord Specialty Dental Partners Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating CDHA Management, LLC and Spark DSO, LLC, which does business as Chord Specialty Dental Partners (“Chord”), regarding its recent data breach. The Chord data breach involved sensitive personal information belonging to an undetermined number of individuals.

Chord is a multi-specialty Dental Support Organization (DSO) based in Tennessee. Founded in 2017, Chord supports the provision of preventive services, diagnostic imaging, and a full range of restorative dental care for its affiliated pediatric practices, orthodontic locations, and ambulatory surgery centers at more than 60 locations across six states.2 Today, Chord’s partner practices include Spark Orthodontics, Children’s Dental Health, Pediatric Dental Associates, Dentistry for Children, Children’s Dental Surgery, and Cumberland Pediatric Dentistry & Orthodontics.3 Headquartered in Nashville, Tennessee, Chord employs over 1,000 individuals.

WHAT HAPPENED?

Recently, Chord announced that it had experienced a data breach in which sensitive personal identifiable information in its care may have been accessed. According to the breach notice, on September 11, 2024, Chord discovered suspicious activity related to an employee’s email account.1 As a result, Chord launched an investigation to determine the nature of the incident.

Through its investigation, Chord confirmed that sensitive personal information in its systems may have been compromised by an unauthorized third party between August 19, 2024, and September 25, 2024. As a result, Chord began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Bank account information
  • Payment card information
  • Medical information
  • Health insurance information

 

On March 14, 2025, Chord posted a notice of the incident on its website. Additionally, Chord will soon begin sending breach notification letters to impacted individuals. Based on the website breach notice, Chord is providing affected individuals with a list of the specific types of sensitive information impacted.
