Holdrege Memorial Homes Data Breach Investigation

Holdrege is a skilled nursing and assisted living facility based in Nebraska. Founded in 1953, Holdrege offers rooms to all residents and provides a wide range of amenities, including a comprehensive activities program, an in-house beauty shop, buffet dining, wireless internet access, transportation services, on-site physical therapy and wellness rooms and a spiritual ministries department.3 Headquartered in Holdrege, Nebraska, Holdrege employs over 150 individuals.

WHAT HAPPENED?

Recently, Holdrege announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, Holdrege recently discovered a network security incident that impacted its systems.1 As a result, Holdrege launched an investigation to determine the nature of the incident.

Through its investigation, Holdrege confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party between October 9, 2023, and November 27, 2023. As a result, Holdrege began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social security number
  • Date of birth
  • Medical information (e.g., medical treatment, procedure, diagnosis, medical record number, medical provider information, medical prescription information, dates of service)
  • Medicaid and/or Medicare number
  • Health insurance claim and/or policy information

 

As a result of the data breach, Holdrege posted a notice of the incident on its website. Additionally, Holdrege has begun sending breach notification letters to impacted individuals. Based on the website breach notice, Holdrege is providing affected individuals with a list of the specific types of sensitive information impacted.
