HCF Management Data Breach Investigation

HCF Management is a senior care network based in Ohio and Pennsylvania. Founded in 1968, HCF Management’s senior care centers provide a variety of services to older individuals, including long term care, hospice care, palliative care, respite care, rehabilitation, memory support, and more.2 Today, HCF Management offers care communities in Ohio and Pennsylvania and assisted living residences in Ohio.3 Headquartered in Lima, Ohio, HCF Management employs over 1,000 individuals.

WHAT HAPPENED?

Recently, HCF Management announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on October 3, 2024, HCF Management learned that a third party had gained access to certain of its computer systems.1 As a result, HCF Management launched an investigation to determine the nature of the incident.

Through its investigation, HCF Management confirmed that sensitive personal information in its systems belonging to patients and residents of its care centers may have been viewed and obtained by an unauthorized third party on September 17, 2024. As a result, HCF Management began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social security number
  • Address
  • Date of birth
  • Phone number
  • Medical treatment information
  • Health insurance information

 

As a result of the data breach, HCF Management and its affiliates posted a notice of the incident on their websites. Additionally, HCF Management recently began mailing breach notification letters to individuals whose information was impacted as a result of the breach. Based on the website breach notice, HCF Management is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
