CODAC Behavioral Health Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating CODAC, Inc., which does business as CODAC Behavioral Health (“CODAC”), regarding its recent data breach. The CODAC data breach may have involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

ABOUT CODAC, INC. D/B/A CODAC BEHAVIROAL HEALTH:

CODAC is a non-profit outpatient provider for opioid treatment based in Rhode Island. Founded in 1971, CODAC provides treatment, recovery, and prevention services to individuals, families, and communities struggling with the challenges of substance abuse, gambling disorders, tobacco use, behavioral healthcare issues, and other addictive or abusive behaviors.3 Headquartered in Cranston, Rhode Island, CODAC has six additional locations and employs over 200 individuals.

WHAT HAPPENED?

Recently, CODAC announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on July 24, 2024, CODAC became aware of suspicious activity in its network environment.1 As a result, CODAC launched an investigation to determine the nature of the incident.

Through its investigation, CODAC confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party during the breach to its network. As a result, CODAC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On December 5, 2024, CODAC completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social security number
  • Date of birth
  • Medical information (e.g., medical diagnosis and treatment information, medical record number, date of birth)
  • Health insurance information

 

As a result of the data breach, CODAC posted a notice of the incident on its website. Additionally, on January 17, 2024, CODAC began mailing breach notification letters to impacted individuals. Based on the website breach notice, CODAC is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
