Shawnee Manor Data Breach Investigation

Founded in 1977, Shawnee Manor provides a variety of services to older individuals, including long term care, hospice care, palliative care, respite care, rehabilitation, memory support, and more.3 Additionally, Shawnee Manor is part of a larger system of senior care communities managed by HCF Management, Inc. Today, HCF Management, Inc. offers care communities in Ohio and Pennsylvania and assisted living residences in Ohio.4 Shawnee Manor is located in Lima, Ohio.

WHAT HAPPENED?

Recently, Shawnee Manor announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on October 3, 2024, Shawnee Manor learned that a third party had gained access to certain of its management company’s computer systems.1 As a result, Shawnee Manor’s management company launched an investigation to determine the nature of the incident.

Through its investigation, Shawnee Manor confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party on September 17, 2024. As a result, Shawnee Manor began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social security number
  • Address
  • Date of birth
  • Phone number
  • Medical treatment information
  • Health insurance information

 

As a result of the data breach, Shawnee Manor posted a notice of the incident on its website. Additionally, on January 9, 2025, Shawnee Manor mailed breach notification letters to individuals whose information was impacted as a result of the breach. Based on the website breach notice, Shawnee Manor is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
