Community Alliance Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Community Alliance, Inc. and its affiliates Community Alliance Rehabilitation Services and Community Alliance Health Partners (collectively “Community Alliance”) regarding its recent data breach. The Community Alliance data breach may have involved sensitive personal information and protected health information belonging to an unknown number of individuals.

ABOUT COMMUNITY ALLIANCE, INC.:

Community Alliance is a health care organization based in Nebraska. Founded in 1981, Community Alliance offers clients integrated health care with a focus on mental health. Additionally, Community Alliance provides services addressing physical health, social wellbeing, individuals’ contribution and purpose.3 Examples of services Community Alliance offers include psychiatric and counseling services, primary health care, rehabilitation services, family education and support, employment and homeless services, an on-site pharmacy, and more.3 Headquartered in Omaha, Nebraska, Community Alliance employs over 200 individuals.

WHAT HAPPENED?

Recently, Community Alliance reported to the Attorney General of the Commonwealth of Massachusetts that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Additionally, Community Alliance posted initial notice of the breach on its website on June 7, 2024. According to the website breach notice, Community Alliance discovered suspicious activity in its computer environment on April 8, 2024.1 As a result, Community Alliance launched an investigation to determine the nature of the incident.

Through its investigation, Community Alliance confirmed that sensitive personal information in its systems may have been compromised by an unauthorized third party between April 2 and April 8, 2024. As such, Community Alliance began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Financial information
  • Medical information
  • Health insurance information
  • Address
  • Birth certificates and/or driver’s licenses
  • Taxpayer or other state or federal ID numbers
  • Employer-assigned identification numbers
  • Online account credentials
  • Electronic/digital signatures

 

Recently, Community Alliance began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Massachusetts residents, Community Alliance is providing affected individuals with a list of the information impacted and 24 months of complimentary identity monitoring services.
